Economic factors and suicidal ideation among metropolitan and rural dwelling elderly in Korea : based on the 2009, 2013, 2017 Community Health Survey

Abstract
Objective: This study was aimed to analyze the relationship between the economic factors of the community elderly (65 years or more) and the suicidal ideation.

Method: Study was conducted on 126,392 persons over the age of 65 who participated in the Community Health Survey (CHS) for 2009, 2013, and 2017 (36,227 in 2009, 43,274 in 2013, and 46,891 in 2017). The survey adopted economic factors including recipients of basic livelihood security, personal equivalized income, and economic activity. Suicidal ideation rate by area (metropolitan and rural) and gender analyzed with chi-square test. Related factors to suicidal ideation were analyzed using complex sample logistic regression analysis. All analyses used SPSS ver. 25 with 0.05 of significance level.

Results: The suicidal ideation rate for the elderly in 2009, 2013, and 2017 were 16.3%, 15.7%, and 11.2%, respectively, which showed a similar level in 2009 and 2013 and decreased in 2017. Based on related factors to suicidal ideation, male seniors who are current recipients of basic livelihood security and female seniors who were ex-recipients of basic livelihood security were high risk for suicidal ideation. From a personal equivalized income perspective, the suicidal ideation in middle income elderly female were decreased, yet increased in low income elderly female regardless of their residential districts. Meanwhile, the number of male seniors with suicidal ideation decreased in metropolitans, yet increased in rural.

Conclusion: Seniors living in poverty were at higher risk for suicidal ideation, but the correlation varied with gender and whether they live in metropolitan or rural. This result is expected to be used as underlying data to develop a suicide prevention policy customized for seniors considering gender and region-specific characteristics
